Housing Stabilization Services (HSS)
We understand that navigating the housing landscape can be challenging, which is why we offer tailored support to help individuals find and sustain housing. At Embrace Care, we provide comprehensive assistance to overcome various obstacles, including securing employment, applying for benefits, and finding suitable housing for every client through the Housing Stabilization Services (HSS) Medical Assistance Program.
Housing Transition Services
We assist clients in planning for, finding, and moving into the home of their choice.
Our covered services include:
● Developing a housing transition plan.
● Assisting benefit applications for client to afford housing and determine which benefits they may be eligible for.
● Assisting with the housing search, application process, screening, and assessments
● Providing transportation for housing-related appointments.
● Helping clients understand
● Finding funding for deposits, organizing moves, and ensuring safe living arrangements.
Housing Sustaining Services
We assist clients in planning for, finding, and moving into the home of their choice. We support clients in maintaining their homes.
Our covered services include:
● Developing and updating housing support and crisis/safety plans.
● Preventing behaviors that may jeopardize housing.
● Educating on tenant and property manager roles, rights, and responsibilities.
● Providing transportation for housing-related issues.
● Supporting applications for benefits to retain housing.
● Helping understand and maintain/increase income and benefits.
● Building natural housing supports and community resources.
● Working with property managers or landlords to promote housing retention.
● Arranging for assistive technology and adaptive accommodations.
Housing Consultation Services
Housing Consultation Services
Embrace Care offers Housing Focused Person-Centered Plans to help support those without case management services find a provider for housing sustaining and transition services. These services include:
● Assisting with accessing documentation required for Housing Stabilization Services eligibility.
● Developing a Housing Focused Person-Centered Plan based on assessment outcomes.
● Identifying the person’s strengths, needs, and housing preferences, including cultural requirements.
● Supporting informed choices in housing transition or sustaining providers.
● Providing resources for non-housing related goals identified in the person-centered planning process.
● Coordinating with other service providers working with the person.
● Helping understand privacy rights and appeal information.
● Annually updating the person-centered plan related to housing
